Hi River Road Parents!
I will be performing a 15‑minute orofacial myofunctional screening with your child at River Road Preschool. Your child’s teacher will notify you of the scheduled time and date.
Please:
List any allergies on the parent surgery form that was sent home.
Check off any concerns on the form and add comments if you would like me to know further details about your child.
During the screening I will observe daily functions such as breathing patterns, posture (walking and bending), and eating and drinking. I will provide crackers and water. If you or your child prefers a different crunchy snack, please send it with your child on the day of the screening and label it: “Myo Screening.”
